You go to bed hoping a night of rest will help. Then you stand up in the morning and your lower back feels stiff, tight, or downright painful.

For many people, the first few minutes of the day are the hardest. They loosen up after a shower or once they start moving, only to repeat the same pattern the next morning.

Morning back pain is common, but it is not something you should automatically dismiss as age, a bad mattress, or “just sleeping wrong.” The way you sleep matters, but so do the hours that come before it.

Why Can Back Pain Feel Worse After Sleeping?

During the day, you change positions constantly. You stand, sit, walk, bend and reach. At night, your body may stay in one position for hours. If your lower back, hips or surrounding muscles are already irritated or restricted, that long stretch of stillness can make stiffness more noticeable when you first get up.

Sleep position can add to the problem. The goal is not to find one “perfect” position for everyone. It is to find a position that lets your spine rest comfortably without forcing it into an exaggerated curve or twist.


Start With the Way You Sleep

A few simple changes may make mornings easier:

  • If you sleep on your back, try a pillow under your knees to reduce the arch in your lower back.
  • If you sleep on your side, place a pillow between your knees to help keep your hips and spine more neutral.
  • If stomach sleeping consistently makes your back or neck hurt, experiment with another position rather than forcing it.
  • Pay attention to your pillow and mattress, but do not assume buying a new mattress will fix pain that keeps returning.


The best sleep position is the one that is comfortable for you and does not aggravate your symptoms.
